Tanzania Health Surveillance Form: New Travel Measures 2026
Last updated: 28 October 2026
Following the Ebola Virus Disease (EVD) outbreak in the Democratic Republic of the Congo (DRC), we want to reassure our guests that Tanzania remains open to international travellers. On 17 October 2026, Tanzania’s Ministry of Health issued Travel Advisory No. 18, adding public health measures for all international arrivals. The main change is the Tanzania health surveillance form, an online form you can complete before you fly, plus a temperature check when you arrive.
Quick Answer
Since 17 October 2026, every traveller entering mainland Tanzania must complete the online Traveler’s Health Surveillance Form at afyamsafiri.moh.go.tz, before departure or on arrival, and keep the confirmation QR code or reference number. All arrivals are health-screened, including temperature checks, at airports, land borders and seaports. Zanzibar has its own form for travellers arriving from or through the DRC, Uganda or Kenya.
Before You Travel: Complete the Tanzania Health Surveillance Form
Every traveller entering Tanzania must now complete the online Traveler’s Health Surveillance Form. You can fill it in before departure or on arrival, but we recommend doing it before you fly to save time at the airport.
- Enter accurate information, as requested by Tanzania’s Ministry of Health.
- Keep your confirmation QR code or reference number, on your phone or printed, as port health officials will check it on arrival.
- Have your passport and flight details ready before you start.

When You Arrive
Port health officials now screen all travellers, including temperature checks, at every point of entry into Tanzania, whether you arrive at an airport, a land border crossing or a seaport. This includes the Namanga border, used on many of our Kenya and Tanzania safaris, so allow a little extra time at the crossing.
During Your Stay
All arriving travellers receive health information from the Ministry of Health and are asked to monitor their own health throughout their visit. If you notice any signs or symptoms of Ebola or another infectious disease, report them to the nearest health facility or point of entry, or contact the authorities:
- Toll-free number: 199
- Email: poe@afya.go.tz
Travellers identified by the authorities as high-risk will be followed up for 21 days through daily automated messages, which continue after they leave Tanzania. All travellers are asked to follow infection prevention measures, including regular handwashing and the use of hand sanitiser. Your driver-guide will be happy to help if you have any concerns while on safari.
Travelling to Zanzibar
Zanzibar’s Ministry of Health has issued its own Ebola-related travel advisory. If you are arriving from or transiting through the DRC, Uganda or Kenya, you must complete Zanzibar’s Traveler’s Health Surveillance Form. Public health officials are screening all international arrivals at Zanzibar’s designated international airports and seaports.
Combining Kenya and Zanzibar? If your trip includes a Kenya safari before Zanzibar, or a Tanzania safari with a Zanzibar beach stay, check whether you need the Zanzibar form as well as the mainland form. Our team will confirm exactly which forms apply to your itinerary.

Onward Travel
The advisory asks travellers leaving Tanzania to check the entry requirements of their next destination and of any country they transit through, especially if your journey continues to Kenya, Uganda or Rwanda.
Staying Informed
The Government of Tanzania will review the advisory as the situation develops, and we will update this page when new guidance is issued. For official information on Ebola, see the World Health Organization. For questions about your booking, please contact our safari team.
